Photo Release Form Template

A photo release form is a document in which a person grants permission for their photos, video or voice recordings to be used for specific purposes, such as marketing, social media or publications. It defines where the media can appear, for how long, whether the person is paid and how they can withdraw consent.

This guide covers what to include, the difference between model and property releases, how to handle minors and how to collect releases online.

What is a photo release form?

A photo release, also called a media release or model release, protects organizations that use images of identifiable people. In the US, many states recognize a “right of publicity,” which limits commercial use of a person’s name or likeness without permission.

Photo releases are used for:

  • Marketing campaigns, websites and ads
  • Customer testimonials and case study videos
  • Events, conferences and webinars
  • School, nonprofit and community programs
  • Employee photos for recruiting and social media
  • Product photography that includes people

Best practices

  • Be specific about uses. “Marketing purposes” is vague. List channels.
  • Separate the release from other forms such as waivers or registrations, or use a separate checkbox.
  • Make it optional where consent is not required to participate.
  • Use clear event notices at conferences, but get individual releases for featured people.
  • Track who opted out and share the list with photographers and social teams.
  • Store releases with the media so marketing can verify consent before publishing.

Collecting photo releases online

  1. Send a release link with event registration, or show a QR code on site.
  2. Conditional logic shows guardian fields for minors and optional uses as checkboxes.
  3. Participants sign on their phone.
  4. A signed PDF is generated and stored.
  5. Consent status syncs to your CRM, digital asset management system or spreadsheet.
  6. Opt-outs are flagged for the marketing team.

What is a photo release form?

A photo release form is a document in which a person gives permission for their photos, video or recordings to be used for specific purposes, such as marketing or social media. It describes the media, permitted uses, duration, compensation, ownership and how to withdraw consent.

Do I need a photo release to post pictures online?

For commercial uses, such as advertising or promoting a product, you generally need permission from identifiable people because of state right-of-publicity laws. Editorial and newsworthy uses are treated differently. When in doubt, especially for marketing, get a signed release.

What is the difference between a model release and a photo release?

The terms are often used interchangeably. A model release typically refers to permission from a person who appears in photos or video, while “photo release” can also describe consent forms used for events or programs. A property release covers private property or artwork, not people.

Can a minor sign a photo release?

Generally, a parent or legal guardian should sign a photo release for anyone under 18. The form should include the minor’s name and age and the guardian’s name, signature and relationship. Schools and youth programs often collect these releases at enrollment.

How long is a photo release valid?

A photo release is valid for the duration stated in the form. Many releases grant rights indefinitely, while others limit use to a specific period. If the form allows withdrawal, organizations usually stop new uses after withdrawal but may not be able to recall published materials.

Can photo releases be signed electronically?

Yes. Photo and media releases can be signed electronically under the ESIGN Act and state UETA laws. Digital releases are easier to collect at events, simpler to search and can be linked directly to the photos or videos they cover.
